The power of metrics in distributed care management
MediCompass® Connect is an FDA-cleared telehealth gateway for members to seamlessly upload biometric data from over 50 personal health monitoring devices from market-leading manufacturers, including glucose monitors, insulin pumps, blood pressure monitors, digital spirometers, pedometers, and weight scales. The easy-to-use system conveniently transfers data via a standard phone line or Internet-enabled PC using our “one-button” MetrikLink® connectivity hub or, for those devices with the capability, connected directly to the PC.
This data is then seamlessly integrated with core population health management systems, such as disease and wellness programs, personal and electronic health records, provider practice tools, predictive modeling applications, etc.

The next generation of disease management—Distributed Care Management—utilizes low-cost biometric data to help extend the reach of traditional models:
- Expand to a broader set of chronic condition program participants, not only the highest risk typically addressed today, so that care management plans can work to prevent individuals from advancing into the high-risk categories.
- Improve predictive modeling accuracy using up-to-date, virtually real-time biometric data to augment claims data, which can lag for as long as six months.
- Focus care management staff on those that need interventions based on virtually real-time, objective data about their condition (e.g., blood glucose variability, PEF and FEV1, blood pressure trends, etc.).
- Increase case loads of care and disease management staff through access to better, more accurate data. Stratify case loads and streamline outreach efforts with population-based biometric alerts and triggers.
Through a Distributed Care Management model, participants are more engaged in the process by actively sharing their objective biometric data directly with physicians, care coaches, and others helping to improve health. This enables customers to capitalize on the core strength of that relationship where care and behavior change are best affected.
